1. Master Subscription Agreement

By creating an account, accessing, or using Remedy ("the Service"), you agree to be bound by these Terms of Service. If you are using the service on behalf of a household with family dependents, you represent that you have legal authority to manage healthcare billing records on their behalf.

2. Scope of Service & Non-Medical Notice

Medical Claim Manager is an administrative financial software application. It audits and reconciles medical provider bills against insurer Explanations of Benefits (EOBs) and patient payments.

Non-Medical / Non-Physician Disclosure: Medical Claim Manager does not provide medical diagnoses, treatment recommendations, clinical opinions, health insurance sales, or formal legal advocacy. All financial recommendations are algorithmic analyses based on in-network insurance contract guidelines and statutory ACA appeal rules.

3. Subscription Tiers & Single Claim Passes

Individual Monthly ($14.99/mo) and Family Plan ($24.99/mo) subscriptions are billed via Stripe and may be cancelled at any time through the customer portal. Single Claim Passes ($49 flat) grant one-time full audit and dispute packet compilation for a single medical episode.

4. User Content & Dispute Packets

You retain full ownership of all medical statements, EOB records, and dispute packets compiled by the platform. You are responsible for ensuring the accuracy of all user-entered billing data before submitting formal dispute letters to hospital billing offices or insurance carriers.

5. Limitation of Liability

Medical Claim Manager shall not be liable for provider credit bureau reporting, collections enforcement, or adverse insurance appeal determinations made by third-party healthcare entities.
